– Complete the form, then enter the total amount of students to calculate payment.
Only school employees, such as directors or school district employees, are authorized to pay audition and participation fees for All-State Groups using a school credit card or P-card. If you are not in this category, please DO NOT PROCEED with this payment. Any unauthorized payments will be canceled.
NOTE: If you need to complete a Petition Form, please remember to complete and send to both Kaitlin Miles and the scheduling governor of the site you are petitioning. You can find the scheduling governor on the Fall 2025 Audition Sites and Dates Document.
If you are requesting Accommodations for students, be sure that you have uploaded completed your IEP/504 Accommodation Request Form(s) and that you have uploaded supported documentation with your registration. If you forgot to upload this with your registration, you will need to reach out to Kaitlin Miles.
