NOTE: If you need to complete a Petition Form, please remember to complete and send to both Kaitlin Miles and the scheduling governor of the site you are petitioning. You can find the scheduling governor on the Fall 2021 Audition Sites and Dates Document.
If you are requesting Accommodations for students, be sure to complete the IEP/504 Accommodation Request Form and send in the supported documentation. This form is to be e-mailed to Kaitlin Miles and the scheduling governor of the site you are registering for. These e-mail addresses are provided on the form.
2. Pay Online:

– Complete the form, then enter the total amount of students to calculate payment.
